Cigna

JPS Health Network offers two Cigna dental plan options for 2027: the Dental Care Access Plus (DHMO+), an in-network-only plan with co-pays per the patient charge schedule, and the Advantage Dental PPO, which allows any licensed dentist with highest coverage in-network and an Annual Preventive Care Reward to increase the annual maximum benefit.

Out-of-Network Notice: For out-of-network services, members pay applicable coinsurance plus any amount that exceeds the usual, customary, and reasonable charge.

Cigna Dental Care Access Plus (DHMO+) Cigna Advantage Dental PPO Plan
  • The plan covers in-network services only.
  • You may visit any dentist of your choice, but you’ll receive the highest coverage when you visit in-network providers.
  • If you visit an out-of-network provider, you will be responsible for the full cost of services.
  • If you visit an out-of-network provider, you will not benefit from discounted rates and will pay more for services.
  • Select a primary dentist who will coordinate your dental care needs, including referrals to specialists.
  • You may pay an annual deductible for select services.
  • You typically pay a co-pay for qualified dental services.
  • The Annual Preventive Care Reward gives you additional money toward your annual maximum benefit when you complete two preventive care exams.

  • PPO: Team members who want flexibility to choose any dentist and value preventive care rewards
Who Can Enroll
  • 👨‍👩‍👧‍👦You may enroll your eligible dependents in many of the same plans you choose for yourself. Eligible dependents include:
